- If Creatinine on Admission ≥5 mg/dL, CreatinineFactor = 10
- Else if Creatinine on Admission ≥3.5 mg/dL, CreatinineFactor = 4
- Else if Creatinine on Admission ≥2 mg/dL, CreatinineFactor = 2
- Else if Creatinine on Admission ≥1.2 mg/dL, CreatinineFactor = 1
- Else CreatinineFactor = 0.
